9:00 AM – 1:00 PM

April 25^th

3:30 – 6:00 PM

Only parents or guardian can register a child.

Items needed for registration:

1.    Blue Form

2.    Social Security Card

3.    Birth Certificate

4.    (2) Proofs of Residency -- Power, Water, Gas

a.     (NO -- cable, cell phone or home phone)

5.    Lease or Deed

6.    Drivers License of parents

7.    Custody Papers / Divorce Papers (If Needed)
